To join, just follow these steps:
- Age Requirement: 18+, or 16+ with parental consent.
- Initial Requirements: Complete an initial cleaning, x-rays, doctor’s exam, and book your six-month re-care appointment.
- Stay Current: Attend hygiene appointments 2 times a year, complete any necessary dental treatments, and maintain a zero balance.
- Get Your Whitening Refill: Receive 2 gel refills per year at your re-care visits.
Terms of Agreement:
- Provide 72 hours’ notice for cancellations.
- Follow the minimum treatment plan as prescribed by the doctor.
- We reserve the right to discontinue your enrollment in the Whitening 4 Life Program if your dental health doesn’t meet minimum standards.

Is the Whitening 4 Life program safe for everyone?
Yes! But if you have untreated gum disease, decay, or other oral conditions, these must be addressed first.
How often do I get whitening gel refills?
You’ll receive up to 2 tubes of whitening gel each year during your regular hygiene appointments.
What if I lose my trays?
Trays can be replaced for $75 if we have your models, or $100 if new impressions are needed.
