Your First Visit

We would like to welcome you to our practice and thank you for choosing us. We look forward to meeting you at your first visit. We offer these tips for your first visit:
  1. Please complete and return the Patient Information Packet forms (one set for each patient) in the return envelope. We would like to receive your paperwork 3 days prior to your appointment to assure we have all of your insurance information updated. Keep the Financial Policy and HIPAA Privacy Policy for your records. You may also download these forms.

  2. Plan to arrive about 15 minutes prior to your appointment time.

  3. Print the driving directions and bring them with you to insure you find us quickly and safely.

  4. If you have dental concerns or questions, write them down and bring them with you. Our dentists and staff are happy to discuss all of these with you.

  5. All children under the age of 18 years must be accompanied by a parent or legal guardian in case of a medical emergency.

  6. Please contact your previous dentist to have current xrays sent to you prior to your appointment with our office. We take Bitewings once each year and a Panorex every three years. If you bring your current xrays to your initial appointment, we may not need to retake or charge you for new xrays.
